Title I School

To tryout for any sport this year you need the following 3 forms completed to stay after school.  All practices are after school until 5:00 pm.

One per year- PHYSICAL Form*

*MUST be on the FHSAA form NOT the Dept. of Health

One per year- MEDICAL RELEASE Form

One per sport- PARENT RIGHTS Form
