To tryout for any sport, you need the following 3 forms completed to stay after school. All practices are after school until 4:45 pm. Practice days are Mon-Thurs. until 4:45 pm
One per year: PHYSICAL Form*
*MUST be on the FHSAA form, NOT the Dept. of Health.
Dept. of Health forms cannot be accepted.
One per year: MEDICAL RELEASE Form
One per sport: PARENT APPROVAL Form
